Turn in as soon as your child has their Dr. appointment!
All medical forms must be turned in by: Date To Be Determined
Medication Collection Day: To Be Determined
Please turn in any medications to the school on/by this day.
(Usually the week prior to school starting- look for further communication).
Please let us know if your child has:
- Asthma
- Diabetes
- Severe Allergies
- Chronic Conditions
- Dietary Needs
- Medications
Please be sure to list all medical conditions, allergies, or other important information on your child’s registration.
Needed Forms and School Supplies for Health Concerns:
On the first day, please be sure your child has 2 pairs of clothes and 2 pairs of undergarments to keep at school just in case!
